Witness Martial Arts Sign-Up

In consideration of being allowed to participate in the activities and programs offered by Witness Martial Arts Gym, I, {name}, hereby acknowledge and agree to the following:

1. Assumption of Risk: I understand that martial arts training involves physical contact and inherent risks. I voluntarily assume all risks associated with participation in the activities and programs offered by Witness Martial Arts Gym. I acknowledge that these risks include, but are not limited to, injury, illness, property damage, and even death.
2. Fitness to Participate: I certify that I am physically and mentally fit to participate in the martial arts training activities at Witness Martial Arts Gym. I have not been advised otherwise by a qualified medical professional.
3. Personal Information: I agree to provide accurate and up-to-date personal information including, but not limited to, my name, {name}, my date of birth {dob}, and my phone number {phone}, and emergency contact information (if applicable). I understand that this information will be kept confidential and used for gym administration and emergency purposes only.
4. Emergency Contact: In the event of an emergency, I authorize Witness Martial Arts Gym to contact the person listed as my emergency contact. I understand that the emergency contact's name is {contact_name}, their phone number is {contact_phone}, and their relation to me is {contact_relation}.
5. Agreement to Follow Gym Rules: I agree to follow all gym rules and regulations as outlined by Witness Martial Arts Gym. I understand that failure to comply may result in my membership being suspended or terminated.

Waiver and Release: I hereby release, discharge, and hold harmless Witness Martial Arts Gym, its owners, instructors, staff, and volunteers from any and all claims, demands, causes of action, liability, loss, damage, or expense arising out of my participation in any gym activities, including but not limited to, personal injury or property damage.

Photographic Release (Optional): I grant Witness Martial Arts Gym permission to use photographs or videos taken of me during gym activities for promotional purposes, including but not limited to, website content, social media, and advertising. I understand that my name will not be used without my explicit consent.

Yay! No long-term contracts and no termination fees. Signing this acknowledgment does not enroll me in a membership, require me to become a member, or authorize any membership charges. This policy will apply only if I voluntarily choose to become a member of Witness Martial Arts.

I, {name}, acknowledge and understand that if I choose to become a member of Witness Martial Arts, I must provide written notice at least 30 days in advance to terminate or cancel my membership. This means that, under normal circumstances, I may be billed one final time before my termination is processed. By signing this acknowledgment, I confirm that I understand the 30-day written notice policy and agree that it will apply if I choose to become a member.

Membership fees must be paid on time, and failure to do so may result in suspension or termination of membership. c. Members should inform the gym of any medical conditions or injuries affecting their ability to participate safely.
2. Code of Conduct: a. All members and staff must treat each other respectfully and courteously. b. Profanity, discrimination, and harassment of any kind will not be tolerated. c. Members must follow gym rules and instructions from instructors at all times. d. Bullying or aggressive behavior is strictly prohibited.
3. Safety Guidelines: a. Proper attire must be worn during class. Any exposure of sensitive body parts is unexpectable, whether intentional or not. b. All participants must adhere to safety protocols and exercise caution to prevent injuries. c. No food, gum, or drinks (except water in closed containers) are allowed on the training floor. d. Members should report any unsafe conditions or broken equipment immediately.
4. Class and Training Policies: a. Punctuality is crucial. Arrive on time for classes and remain for the entire duration. b. Keep noise levels to a minimum during classes to avoid disrupting others. c. Use equipment responsibly and return it to its proper place after use. d. Only qualified instructors are allowed to teach classes.
5. Hygiene and Cleanliness: a. Members are expected to maintain good personal hygiene and wear clean training attire. b. Gym equipment should be wiped down after use, and members should clean up their training area. c. Shoes SHALL NOT be worn on the training floor.
6. Personal Belongings: a. The gym is not responsible for lost or stolen items. b. Valuables should be left at home or secured in vehicles.
7. Injuries and First Aid: a. In the event of an injury, members should report it to staff immediately. B. Emergency contact information should be up to date for all members.
8. Cancellation and Refund Policy: a. Members should adhere to the cancellation policy for classes and private lessons.
9. Social Media and Publicity: a. Members need not obtain permission before posting photos or videos taken in the gym on social media or for promotional purposes.
10. Dispute Resolution: a. Any disputes or concerns should be addressed with gym staff.
11. Temporary image rights: a. By signing this document I give witness martial arts permission to use my image for social media and promotional purposes.
12. Amendments to Policies: a. The gym reserves the right to update and modify policies as needed. Members will be informed of any changes.
